Admissions
Referral for consideration for admission to our homes is primarily coordinated with your Tri-Counties Regional Center Service Coordinator.
For Intermediate Care Facilities, regulations specify the following eligibility criteria for admission.
ICF/DD-Habilitative
Must have two developmental deficits in either of these domains:
Self Help Domain
- Eating
- Toileting
- Bladder control
- Dressing
Social-Emotional Domain
- Unacceptable social behavior
- Aggression
- Self-injurious behavior
- Smearing
- Property destruction
- Runs or wanders away
- Temper tantrums
Conditions which may exclude beneficiaries from placement:
- Aggression – has had violent episodes which have caused serious physical injury in the past year.
- Self-injurious behavior causes severe injury which requires physician attention at least once per year.
- Smearing-smears at every opportunity
- Has an active communicable disease that is required to be reported
- Has a decubitus of any stage.

Must have two developmental deficits in any one or combination of these three domains:
Self Help Domain
- Eating
- Toileting
- Bladder control
- Dressing
Social-Emotional Domain
- Unacceptable social behavior
- Aggression
- Self-injurious behavior
- Smearing
- Property destruction
- Runs or wanders away
- Temper tantrums
Motor Domain
- Ambulation
- Crawling and standing
- W/C mobility
- Rolling and sitting
Must have need for intermittent skilled nursing services such as:
- Apnea monitoring
- Colostomy care
- Gastrostomy feeding and care
- Naso-gastric feeding
- Tracheostomy care and suctioning
- Oxygen therapy
- I.P.P.B.
- Catheterization
- Wound irrigation & dressing
- Licensed nurse evaluation on an intermittent basis – see eligibility guidelines
- Special feeding assistance
- Repositioning to avoid skin breakdown which would lead to decubitus ulcers and contractures.
Conditions which may exclude beneficiaries from placement:
- Aggression – has had violent episodes which have caused serious physical injury in the past year.
- Self-injurious behavior causes severe injury which requires physician attention at least once per year.
- Smearing-smears at every opportunity
- Has an active communicable disease that is required to be reported
- Has a decubitus of stages III or IV.
